YAppSurvey enables creating custom questionnaires to collect attendee details during ticket purchases, usable in both Box Office and Online Ticket Store events. The guide covers logging into Yapsody, creating questions, building and saving questionnaires, linking them to events, and configuring settings like mandatory completion. Once linked, questionnaires cannot be edited after transactions begin.

Step 1 — Open YAppSurvey
- Log in to your Yapsody account.
- Click your Organization Name in the top-left corner.
- Select YAppSurvey from the dropdown menu.
- Click Create/Manage Questionnaire.
Step 2 — Create Custom Questions
- Open the Question Bank tab.
- You will see a list of default questions already available.
- Click Add New Question to create your own question.
- Enter your question details.
- Select the appropriate response format.
- Mark the question as Mandatory if attendees must answer it.
- Click Save.
Step 3 — Create Your Questionnaire
- Open Questionnaire Builder.
- Click Create/Add Questionnaire.
- Enter a questionnaire name that is easy to recognize later.
- Click Save.
You will now see all questions from the Question Bank on the left side of the screen.
Step 4 — Add Questions to the Questionnaire
- Add questions to your questionnaire using:
- Drag and drop
- Attach option
- Add one question at a time.
- Arrange the questions in the desired order.
- Click Save once all questions are added.
After saving, your questionnaire will appear in the questionnaire list.
Step 5 — Link the Questionnaire to Your Event
- Click Back to Box Office.
- Select the event where you want to use the questionnaire.
- Click Edit.
- Open the Customize tab.
- Select YAppSurvey.
Under Own Questionnaire, you will see the questionnaires you created earlier.
You can link the questionnaire to:
- Online Store
- Box Office
- Both
Additional Settings
You will also see additional options such as:
Allow answers to be duplicated for all attendees
Allows ticket buyers to copy the same answers for multiple attendees.
Make YAppSurvey mandatory
Requires attendees to complete the questionnaire before checkout.
Save Your Changes
Once your questionnaire is linked and configured:
- Click Save and Proceed.
- Your questionnaire is now active for the selected event.
Important Note
Once a transaction has been attempted for an event, the linked YAppSurvey questionnaire cannot be edited.
